It looks like the kid has something smeared on his tongue. They probably just trained the lizard to eat a type of paste, and then just smear it where ever they want it to lick.

From purely an artist's point; there's no clear signs of it being Photoshoped. Whoever took it must've been pretty lucky!
Chameleon tongues snap out and back pretty quickly in order to catch flying insects. Usually higher speed film is used to capture this. This image shows absolutely no blurring whatsoever. I vote fake.

High speed film is required to capture the tongue in motion, but a good 35mm should have enough shutter speed and exposer settings to capture this type of still
I just don't think so. Even with a decent camera there would still be some movement evident at the speeds their tongues move. And to have captured just that split second that it happened seems unlikely. I still say fake.

Well, if it is photoshopped, then it's a very good job. I can't see any signs of it. So I'm inclined to say that it is an actual unaltered photograph.
Whether what it appears to depict is what is actually happening, though, I haven't any real idea. There appears to be a mealworm or something similar on the boy's tongue, so I suppose it is possible that the lizard is going after that. It wouldn't be much different from many other people who feed their cats or dogs or birds in similar manner. The photographer could have set up some sort of high-speed camera to take a series of pictures, like in those pictures you'll sometimes see of a flying bullet. Then they could use the one picture of the series that shows the actual contact taking place.
Or there could be something else going on. The "tongue" might actually be some sort of fake, with one end balanced on the boy's tongue and the other being bitten by the chameleon. Or the entire chameleon could be a good fake. Or it could be something else altogether different

I've tried 4 different times...can't get it to load...I'm really intrigued now.
